Dental Implants, Periodontists or Dentists only?

I had dental implants and bone grafting done on teeth 7/8 on Wednesday, Jan. 10. Both teeth were pulled at the same time. My concern is, when I took the flipper out on Thursday after 24 hours to rinse with salt water, the site was bleeding a bit. It appears that the suture gave in one spot. Will this affect the turn out of my implants? Will they fail Have you delt with this before? My periodontist only works in my office on Wednesday's so I've been unsuccessful in reaching him. I've gone through enough already and I just don't want to have them fail. Last March I had them done on teeth 9/10 and they are working out well. I didn't have this happen with them. HELP me as I'm worried sick until my next appointment on Wednesday, 1/17. I'm taking amoxicillin daily 4 times too. I also


didn't have much swelling from this as I did last year. Is this normal? Last year on teeth 9/10 it felt like someone one hit me across the face. This time it did not. Let me know your thoughts.

i will try my best to answer your ?'s: I am a dental assistant, and although I only work now in an office that does root canals, I have worked some with implants, and much with extractions/prosthetics in the past. Sometimes there can be a bit of bleeding 24hrs after an extraction, esp. since you have a flipper in place. While you were sleeping that night you could have accidentally bumped the area or clenched your teeth too tightly, causing the already sore area to bleed a little. Keep taking the abx, and everything should be fine. If you have noticeable swelling or anything changes that you are not comfortable with then you should try to get in touch with the office. How have the implants been on #9 %26amp; #10? As long as you do everything they tell you to the implants you just had placed should do well for you! Hope I helped!
Reply:I am the implant coordinator in my office. I dont think you are going to have any problems with the implants just because their was some bleeding or because the suture was a little loose.





The main concern is the implant integrating in the bone. I know every dentist is different, and I'm not downing the guy who did yours, but my dentist doesn't think it is a good idea to use a flipper as the temporary. He has said that a flipper can cause the tissue to not heal properly around the implant and make it difficult to have a permanant crown made that looks very natural around the gums. We usually just make a temporary crown to go on the implant when it is a front tooth, and shape the underneath side of the temp so that the tissue heals as perfect as possible.





But each dentist is different so i am sure yours had a reason for doing the flipper instead of the temporary crown.





If precausions are taken, such as the antibiotics and steriods like we do in my office, there doesn't have to be any swelling.

Reply:If there is visible tissue that has come loose from a suture, and the suture has come undone, then you may be at risk for losing the bone graft that was placed, depending on what materials were used. The implant should not be at much risk of "falling out", but may fail if there is sufficient bacterial insult. These are worst case scenarios .





More than likely there is just a small bleed from an area that was maybe not closed all the way with the sutures, and your sutures are still intact. Keep the flipper in place as you were told to do so as this will help create pressure to stop the bleed and keep the tissue together. If you are still bleeding (and I dont know why this works) try placing a tea bag on the area. My old boss used to do this and it seemed to work great. Keep trying to get in touch with your periodontist, or if you are unable and still worried, try contacting your dentist.
